US Payroll - Government Forms
The Government Forms menu is used to generate various US governmental taxation forms.
NOTE: Many of the forms available in the US Payroll module are documented in
Annual Fed 940
Form 940 is used to report annual Federal Unemployment Tax Act (FUTA) tax. This tax provides funds (along with the relevant state unemployment tax system) for unemployment compensation for workers who have lost their jobs.
This form is not printed through Payroll, rather an XML file is created which can be imported into a third-party program such as Greenshades.
941 Filing
Form 941 is used to report income taxes, social security tax, or Medicare tax withheld from employee paycheck, as well as pay employer portions of social security or Medicare tax.

W-2 Form
The W-2 form is used to report employee wages and the taxes withheld from them. A form must be filled out for each employee paid a salary, wage, or other form of compensation as a result of being employed. For more information, please refer to Create W-2 Export File for Greenshades.
This form is used in conjunction with the Greenshades Locality sub-menu where work locations are mapped to locality and municipality codes, mappings are specified for FEINs, W-2 forms are named for specific locality codes, and W-2 mappings for multi-localities can be overridden. NOTE: The W-2 files generated by this screen are formatted as txt files. Greenshades uses XML files so please ensure that W-2 files have the correct file extension before they are entered.
SUI Magnetic Media
Enter the state unemployment information. This works together with the 940 form to calculate unemployment compensation.
401K
Use the 401K sub-menu to set up 401k plan codes, map box codes to CMiC transaction codes, and create 401K files that can be imported into Greenshades.
Greenshades Locality
Use the Greenshades Locality sub-menu to customize fields on W-2 forms and establish relations and mappings between work locations, locality codes, and municipality codes already defined in the Payroll module. For more information, please refer to Greenshades Locality Options.
