Human Resources - EEO 100 Employee Status Report
The EEO-1 report is a federally mandated survey requiring private employers and federal contractors to state their employment data for each site location. Completing this form is required for all private employers subject to Title VII of the Civil Right Act with 100 or more employees

By default, this is a consolidated report for the entire company, but it is possible to restrict it to include information for a single region.
Company
Enter/select the company code.
Report Class ID
Enter/select the report ID. These codes can be set in the Map Positions to EEO Classifications screen (standard Treeview path: Human Resources > Setup > Local Tables > Position/Occupation Information > Map Positions to EEO Classifications).
Address Code
Enter/select the address code for the company.
Region
Enter/select the region in which the company operates.
From Date, To Date
Enter the date range for which the report should be printed.
An EEO-1 report was previously filed for this establishment – Checkbox
Check this box if an EEO-1 form has been filed for this company at any point in the past.
This Company has at least 100 employees in the reporting payroll period – Checkbox
Check this box if the company meets the minimum employment threshold for required EEO reporting.
Affiliated in an enterprise with total employment of 100 or more – Checkbox
If the company is run as part of an enterprise which has, in total, more than 100 employees then check this box.
This Company or one of its establishments fulfills conditions from section C-3 – Checkbox
If the answer to the question asked by section C-3 on the EEO-1 report is “Yes”, check this box.
Duns Number
Enter the nine-digit Data Universal Numbering System (DUNS) number. These numbers are used to monitor the dispersion of federal grant money.
Major Activity
Enter the product or general group of products on which the greatest number of employees work for the company.
